Registration

As a parent of a child enrolled in Open Door Preschool, I understand my responsibility to my child's school and willingly agree to the expectations outlined below and included, but not limited to, this Parent Handbook.

Permission is granted for my child to be included in routine evaluations. As part of this program, permission is granted for observation of children by the Director to provide feedback about ways to improve and make your child’s experience optimal.

I hereby grant permission for Open Door Preschool/Open Door Baptist Church Ministries to use photos or quotations involving my child in school promotional materials or on the school Facebook account. No personal information will be published in promotional materials without the express consent of the parents.

I give my permission for Open Door Preschool to publish my phone number and email address solely for distribution to my child's classroom roster.

I grant permission for my child to use all playground equipment and participate in all preschool activities on the campus of Open Door Baptist Church. I understand that in the event of an emergency, the school and its employees and agents will call emergency services and contact me as soon as possible. I give consent to any licensed physician to administer drugs or medicine or to perform such medical procedures as that physician determines necessary for the relief of pain and to preserve my child's life or health. I understand that I am responsible for all medical transportation, rescue and other related expenses incurred on behalf of my child.

Your child's safety is our number one priority. Our staff may request information from you and other authorized parties to verify identity and ensure the safe pick up/delivery of your child to preschool. As parent/guardian of your child, we expect you to relay our policies to others who may interact with our staff on behalf of your child (daycare providers, relatives, etc.)

All children enrolled in our preschool must be toilet trained prior to attending. Pull-ups or diapers are not to be worn. Children should need no assistance or reminder to use or how to use the restroom. If an “incident” should occur, parents will be notified. Please understand, if your child is not completely toilet trained, we will provide the option to place your child on a waiting list until he/she is completely toilet trained.

The parent or guardian is responsible for the child’s tuition. Tuition is calculated yearly and varies depending on the option you choose. Our yearly tuition is divided in 11 equal payments August through June payment is due on the 5th of the month (first payment August 5th -Last payment June 5th) a $50 late fee will be added to your tuition if paid after this date. There will be a $50 charge for all checks or credit/debit card transactions declined for any reason. All payments must be made through the Push Pay App to Open Door Preschool. Unfortunately, we are not able to pro-rate monthly tuition. School holidays and state mandated breaks are not subject to prorating or reimbursement. In order to qualify for enrollment the registration fee $150 (per child) and the curriculum fee $150 (per child) must be paid in full. By signing, I agree to pay for the Tuition for the current school year.

I release and agree to hold harmless, defend and indemnify, the school and its director, officers, employees and agents from and against any and all claims for personal injury (including loss of life) and all other losses or damages (except those caused entirely by the gross negligence or intentional conduct of the school) that the child or the parent may suffer as a result of the child's participation and/or enrollment at the preschool.
